Patients with RA or fibromyalgia at risk of long-term opioid use

Image
UK: One in three patients with rheumatoid arthritis or fibromyalgia who take opioids for the first time may be at risk for long-term opioid use, findings from a research letter published online in the Annals of the Rheumatic Diseases have shown. The findings imply the vulnerability of patients with rheumatic and musculoskeletal conditions to long-term opioid use.  People with rheumatic and musculoskeletal conditions are often prescribed opioids to manage their pain, and a proportion of them will become long-term users with the attendant risks of dependence and harmful side effects, point out the authors. Most research defines long-term opioid use as 90 or more days, although definitions vary, and there are no contemporary estimates of the scale of long term opioid use, they add. To assess the proportion of patients transitioning to long term use among those newly started on an opioid, they drew on the anonymised medical records of 841,047 adults whose details had been entered...

PIM-3 score and abnormal serum sodium levels related to development of AKI

Sodium and chloride disturbances have attracted increasing attention in recent years. Many pathophysiological effects are associated with hyperchloremia, including reduction in mean arterial pressure and acute renal disease. PIM-3 score and abnormal serum sodium levels are correlated with AKI development suggests a new study published in the BMC Nephrology Sodium and chloride disturbances have attracted increasing attention in recent years. Many pathophysiological effects are associated with hyperchloremia, including reduction in mean arterial pressure and acute renal disease. Pediatric patients undergoing liver transplantation are at risk of developing various electrolyte and biochemical abnormalities, with an impact on their postoperative outcomes. A study was conducted to analyze the impacts of serum sodium and chloride levels on prognosis of Pediatric Liver Transplant receptors. This was a retrospective analytical observational study performed in a single transplant reference...

A Novel Therapeutic Approach for Managing Hidden Epidemic of FLA1c in Patients with T2DM

Image
Non-alcoholic fatty liver disease (NAFLD) affects up to one-third of the general population worldwide. Type 2 diabetes mellitus (T2DM) and obesity are present in more than half of these patients. Ketamine nasal spray may prove safe and effective treatment for refractory migraine

Image
Ketamine taken in the form of a nasal spray may prove a safe and effective treatment for refractory chronic migraine, suggests a single centre study, published in the open access journal Regional Anesthesia & Pain Medicine . It’s a more convenient alternative to intravenous infusion-the usual method of administration for these patients-but the potential for overuse means that it should be reserved for those in whom other treatment approaches have failed, caution the researchers. Several clinical trials have shown that intravenous ketamine is effective for chronic headache. But it typically requires the input of a pain specialist to adjust the dose and monitor side effects, so limiting its use in outpatient clinics, say the researchers. And while there is some evidence that intravenous ketamine can be used for headache disorders, such as migraine and cluster headache, it’s not clear if ketamine could also be safely used for chronic migraine that is resistant to treatment. To tr...

Blood biomarker to predict future Alzheimer’s disease

Image
Why do some people develop Alzheimer’s disease while others don’t? And, even more puzzlingly, why do many individuals whose brains are chock-full of 1 toxic amyloid aggregates-a telltale sign of Alzheimer’s brain pathology-never go on to develop Alzheimer’s-associated dementias? 2University of Pittsburgh School of Medicine researchers appear to have found the answer. Star-shaped brain cells called astrocytes are key to swaying the pendulum in Alzheimer’s disease progression, shows new game-changing research published today in Nature Medicine. By testing the blood of more than 1,000 cognitively unimpaired elderly people with and without amyloid pathology, the Pitt-led research team found that only those who had a combination of amyloid burden and blood markers of abnormal astrocyte activation, or reactivity, would progress to 3symptomatic Alzheimer’s in the future, a critical discovery for drug development aimed at halting progression. Scientists tested blood samples from participan...

Novel endoscopic imaging system can detect multiple fluorescent tracers

Image
Endoscopic surgery is one of the primary treatment options to remove tumors for patients with solid cancers. However, there is a high risk of cancer recurrence if even a small number of cancerous cells are left behind after surgical resection. To prevent this from happening, researchers developed fluorescence-guided surgery (FGS). In FGS, patients are injected with a fluorescent probe that preferentially binds to tumor cells, enabling surgeons to easily identify lesions with the help of specialized endoscopes that emit the necessary excitation light. Unfortunately, tumors can be highly heterogeneous, and a single fluorescent probe is not enough to detect them all. Thus, one of the frontiers in FGS is using cocktails of multiple fluorescent probes (a.k.a. “tracers”) to detect a wider range of tumors, as well as to reduce false positives and negatives. Despite some progress in this direction, clinically approved endoscopes are all optimized to detect only a single tracer. Moreover, mul...

Role of previously unknown protein uncovered in obesity and diabetes

Image
A research team from Boston University Chobanian & Avedisian School of Medicine has identified a novel druggable signaling molecule involved in obesity, a previously unknown protein (MINAR2) discovered in 2020 in the laboratory of Associate Professor of Pathology and Laboratory Medicine Nader Rahimi, PhD. More than 40% of Americans are considered obese, and the trend continues to grow. The treatments or preventive options for obesity and obesity-associated diseases are limited. It is a major national healthcare and public health burden significantly increasing the risk of diseases such as diabetes, hypertension, and cancer and is linked to the severity of COVID-19. “This finding can help to unravel new aspects in the mechanisms of obesity and diabetes, which could lead to the development of novel therapeutics for the prevention and treatment of obesity and diabetes,” said Rahimi, a corresponding author of a paper Inactivation of Minar2 in Mice Hyperactivates mTOR Signaling and R...

Novo Nordisk gets EMA panel recommendation for once-weekly Sogroya for expanded use in children, adolescents with growth hormone deficiency

Image
Denmark : Novo Nordisk has announced that the European Medicines Agency’s (EMA) Committee for Medicinal Products for Human Use (CHMP) has adopted a positive opinion, recommending once-weekly Sogroya (somapacitan) for replacement of endogenous growth hormone (GH) in children aged three years and above, and adolescents with growth failure due to growth hormone deficiency. Symptoms of growth hormone deficiency in children often start early in life, leading to short stature as well as other growth-related health problems. Current treatment for growth hormone deficiency typically requires daily injections of growth hormone. However, frequent injections can be burdensome, disrupting daily life, reducing adherence and thereby affecting clinical outcomes – particularly in children. “The positive CHMP opinion is a step forward to reducing the impact of growth hormone deficiency for children, adolescents and their families in Europe,” said Martin Holst Lange, executive vice president for Dev...

Is sagittal split or intraoral vertical ramus osteotomy better for improving long jaw patients’ QoL?

Image
Hong Kong: Sagittal split ramus osteotomy and intraoral vertical ramus osteotomy both improve quality of life (QoL) in the long term among patients with mandibular prognathism, shows a study published in Clinical Oral Investigations. However, the researchers noted earlier improvements in mental and oral health-related QoL of SSRO groups.  With a normal bite, the upper jaw and teeth sit slightly before the lower jaw. However, when people are described as having a long face or a prominent jaw, they may have a condition called mandibular prognathism which refers to the overdevelopment of the lower jaw. The condition is a facial deformity and affects the chewing function, facial aesthetic, and self-esteem of the individual, as well as causing jaw joint pain. Mandibular prognathism is a common facial deformity in Hong Kong and Southern China compared to Western countries. Correcting mandibular prognathism requires orthognathic surgery, which involves creating a split in the jawbone(...
